For clients new to Healthstyles, you will need to complete our New Request for Service Form.

This form assists us to determine the support required by a presenting participant.

To access our Specialised Behaviour Services, you will need to endorse Healthstyles Clinic as a provider. Click here to find out more about the Endorsement process.

Once the intake process has begun, we will be in contact to find out more about your needs, and begin planning your support outcomes
